Family Planning Together

Here are three conversation formats that might be especially useful in training and with Families Planning Together.  All are in Adobe.pdf format for downloading. Here is one for the individual, another for family members and support services, and here is one for relatives and friends.

Listen to Me! is a way to get started in helping someone move towards the life that he or she wants to lead. It can be used by individuals and/or family members as a way to get ready for an ELP or individual service planning session. The workbook gives people a chance to think about the kinds of things that are important in their lives. It's in Adobe.pdf format.
